Cancellation Forms with PHP

Hi!
This seems like a fairly simple question, but I haven’t been able to track down the answer anywhere. How can I add in an s2member ProForm via php, rather than through the shortcodes provided? I need to insert the cancellation form into a page template, so the shortcode is not going to work.

You can still use the shortcode in the template like this:

<?php echo do_shortcode("[shortcode]"); ?>
