Checkout Options Not Working

I use this function on two other websites and everything is fine. I am having a very difficult time identifying the problem. I recently rebuilt this website from the ground up, so maybe I missed a setting this time around with s2Member.

I have used this previously, and it worked fine, and, I use it exactly like this on another site with the same theme. I have turned off all plugins, still happens. I tested it with the default WP theme, still happens.

At the top of the form, there is a non-functioning “Checkout Options” menu and halfway down, the “Billing Method” has non-functional radio boxes.
