Offer different "products"

Hi there,

I might have missed something but I´d be happy if you could push me into the right direction. What I want to do is this:

I have two pages let´s call them A and B
I offer access to either page A or B for a month, six months or one year (so 6 variations of memberships)
I want to use the PayPal pro forms because I want to offer credit card payment as well

When I generate the shortcodes for these variations I get six different pro forms, right? Or am I getting something wrong because if that would be the case it wouldn´t make sense.

What I would like to have is one form that enables the user to select his desired membership fill out the form and pay (either by paypal account or credit card).

How can I do that?
