I think I have problem with CSS. The problem is not with some plugin, because I stopped all plugins and problem is still there. I’m sure the problem is in my theme, but I want to fix it and to use s2member.

Hi Alex.

Rather than a CSS issue, I’m thinking it may be a JavaScript one.

Could you first confirm that it’s the theme? Test it quick changing to the default TwentyEleven theme and refreshing the page with the pro-form.

The problem may be that the theme is not enqueueing the JavaScript properly. http://codex.wordpress.org/Function_Reference/wp_enqueue_script

I’m sure, the problem is in theme. Unfortunately, I’m not so good with java coding :(

Thanks for confirming it.

I’d contact the theme’s developer asking him for help to resolve this. It may not be much trouble for him. It’d be good to confirm with him if he’s sing [hilite mono]wp_enqueue_script[/hilite].
