Registration Page Still Not Showing UserName

The Username no longer appears on my Registration Form. I am a pro user. It only shows email, first name, and last name, then the captcha. When users hit Submit, it says that a Username is needed, but there is no field for one????

I tried changing the registration to Open, even though I do not want it to be open, and it still does not show a UserName field. It always worked before, now it has stopped. I can give someone my login details, but I need a private email to send the information to.

I sent my login details before to S2Member in response to another trouble ticket, but never received any help.

Very sorry for overlooking your previous ticket. We receive a lot of Private Messages and we generally look for a reply on threads saying that they submitted the form rather than looking at the thread tied to the message. We can continue the issue here.

I took at your registration page and s2Member is in fact generating the Username field here. Something on your site is causing it not to show up. Can you please try the first 2 steps here, please?
