If you leave the password fields blank you get the following message as part of the JS alert “Password (type this twice please) * This is a required field, please try again.” but if you complete both fields but the passwords don’t match there is no JS warning message. Is this the way it works or am I experiencing some problems?

You should be getting a mismatch warning. Please try these to help you find what’s causing it to not work correctly: Knowledge Base » Common Troubleshooting Tips

OK, I’ll have to switch to auto-generated passwords. There is no JS error so its almost impossible to debug, I deactivated all plugins except one, Advanced Custom Fields, as its critical to the function of the site. Thanks
