Sign up boxes disappear

I run a membership site that sells stock buy and sell signals for 7 broad market indexes. I have a sign up page that gives two pricing options (www.stock-signal.com/sign-up/). When you select one, it then displays sign up boxes for data input tied to a Paypal sign up via S2 Member.

Just yesterday, I noted that the sign up boxes have disappeared when you select a payment option and in their place is the S2 Member code as text on the page.

Do you have any suggestions what might be causing the sign up boxes to disappear and be replaced by the S2 Code in Text format?

My site developer no longer works with us and I have been left to deal with this issue on my own. Thanks….Jeff

Are the signup boxes down at the bottom of the page?

No, they are in the middle of the page after two radial buttons to choose your pricing option. Thanks in advance for any help.

Make sure you paste the shortcode into the post/page in VisualMode and not Text mode, I have seen that be an issue. You can also try implementing the TRUEedit plugin
