Thanks. I had also posted this question to the creator of the theme I was using and they said to add this to the custom CSS for the theme:

div.s2member-pro-paypal-form-section>div.s2member-pro-paypal-form-div label { display: block !important }

It appears to have worked.

I am adding this here in case anyone else runs into similar issues.

Update: I changed to the default twenty twelve template and the fields showed up. There is something in the theme that is hiding the form fields.

I just read the installation instructions for Pro, so question #1 about upgrading is clear.

Still need any feedback/answers for question 3 and 4 – mostly question 3 about having multiple registration forms that assign people to specific custom capability groups automatically. That functionality would make the Pro version work for me.
